Love Pizza, a takeaway at 166 Ormeau Road, Belfast was given the score after assessment on June 29, the Food Standards Agency's website shows.
It means that of Belfast's 392 takeaways with ratings, 190 (48%) have ratings of five and just three have zero ratings.
